Discharge Education (Hospital to Home)
I am looking to revamp our discharge educational material. In the past, we have used a “map” or “train” model. We would like to modify this to be less chronological because at times families struggled with feeling like they were not progressing through the map in the “right” order. Can anyone share resources that work well for helping teams and families navigate complex cardiac discharges? We are open to having separate materials for single vs dual ventricles, etc. Thanks, Jessica (I am an Acute Care Cardiac Educator but will be working with the PICU Educators with an end goal of creating a physical document that will follow the patient as they transition from hospital to home)
